New Help For Diabetes Sufferers

The Patient Education Program of Community Care Physicians, P.C., (formerly WE Care) and CCP Diabetes and Nutrition Center, is conducting a program that offers four key services to help patients achieve and maintain a healthy lifestyle. These include medically supervised weight management, medical nutrition therapy, diabetes education and a series of lifestyle improvement classes. This multifaceted program provides a comprehensive approach to wellness in addition to a structured, supportive environment to help patients achieve long-term health and wellness.

Providers include Rachel Colfer, PA; Linda Katz, FNP; Laurie Burton-Grego, MS, RD, CDN, CDE; Catherine Dascher, RD, CDN, CDE; Lisbeth Irish, RDN, CDN, CDE; and Marie Occhiogrosso, BSN, RN, CDE.

Wellness is more than being free from illness; it is the spirited process of growth and change that lasts a lifetime. It encompasses the overall balance of physical, mental and spiritual health. If we take responsibility for our own health and wellness, we can dramatically improve our quality of life.

Most services provided by the Patient Education and Wellness Program are covered under most major health insurance plans.
